We report a rare cause of gastrointestinal bleeding in two infants under 6 months of age, both presenting with hematemesis and melena without other apparent skin lesions. Endoscopic examinations revealed flat, diffuse, active mucosal bleeding due to petechial lesions of the fundus in one infant and of the antrum in the other, suggestive of congenital gastric angiomatosis. The first patient received systemic hemostatic therapy and gastric protection with good clinical outcome. The second patient, who had a more persistent bleeding syndrome, was treated with hemostatic therapy, gastric protection, a proton pump inhibitor, and a beta-blocker, with good clinical resolution of symptoms.
Nous rapportons une cause rare d’hémorragie gastro-intestinale chez deux nourrissons de moins de 6 mois présentant tous des hématémèses et des mélénas sans autres lésions cutanées apparentes. Les explorations endoscopiques ont permis d’objectiver un saignement actif muqueux plan, en nappe, dû à des lésions pétéchiales du fundus chez l’un et de l’antre chez l’autre évoquant une angiomatose gastrique congénitale. Le premier patient a bénéficié d’une prise en charge à base d’hémostatique par voie générale et de pansement gastrique avec une bonne évolution clinique. Le second patient, qui a un syndrome hémorragique plus persistant, a été mis sous hémostatique, pansement gastrique, inhibiteur de la pompe à proton et bêtabloquant avec une bonne évolution clinique des symptômes.
