Abstract

Introduction Hamman syndrome corresponds to spontaneous pneumomediastinum associated with subcutaneous emphysema. It is a rare condition, often underdiagnosed in our setting due to the nonspecific nature of its symptoms, particularly in the obstetric setting. Case report We report a case of Hamman syndrome in a patient managed in the pneumology department of Yalgado Ouédraogo University Hospital. The patient was a 19-year-old woman, on day 11 after cesarean section, admitted for grade IV dyspnea on the mMRC scale, with oxygen desaturation to 85% on room air, no signs of respiratory distress, and cervicothoracic subcutaneous emphysema. Thoracic CT angiography showed pneumomediastinum associated with alveolar syndrome. Management combined oxygen therapy, nefopam, and amoxicillin-clavulanic acid, with a favorable outcome. Conclusion Hamman syndrome is a rare condition. Clinically, it should be considered in any case of acute dyspnea and/or acute chest pain associated with subcutaneous emphysema. Contrast-enhanced chest CT is the reference examination for confirming the diagnosis and should be performed early to avoid unnecessary invasive investigations. Management is most often symptomatic, with a favorable outcome.

Résumé (French)

Introduction Le syndrome de Hamman correspond à un pneumomédiastin spontané associé à un emphysème sous-cutané. Il s’agit d’une affection rare et souvent sous-diagnostiquée dans notre contexte, en raison de la non-spécificité de ses symptômes, en contexte obstétrical. Observation Nous rapportons un cas de syndrome de Hamman chez une patiente prise en charge au service de pneumologie du CHU Yalgado Ouédraogo. Il s’agissait d’une patiente de 19 ans, à J11 post-césarienne, admise pour une dyspnée stade IV de la classification mMRC, avec une désaturation en oxygène à 85 % à l’air ambiant, sans signes de lutte ventilatoire, et un emphysème sous-cutané cervico-thoracique. L’angioscanner thoracique a objectivé un pneumomédiastin associé à un syndrome alvéolaire. La prise en charge a associé une oxygénothérapie, du néfopam et de l’amoxicilline-acide clavulanique, avec une évolution favorable. Conclusion Le syndrome de Hamman est une pathologie rare. Cliniquement, il doit être évoqué devant toute dyspnée aiguë et/ou douleur thoracique aiguë associée à un emphysème sous-cutané. La TDM thoracique injectée est l’examen de référence permettant de confirmer le diagnostic ; elle doit être réalisée précocement pour éviter des explorations invasives inutiles. La prise en charge est le plus souvent symptomatique, avec une évolution favorable.
